RODO

In fulfillment of the information obligation set forth in Art. 13 para. 1 and 2 of Regulation (EU) 2016/679 of the European Parliament and of the Council of April 27, 2016. on the protection of individuals with regard to the processing of personal data and on the free movement of such data and the repeal of Directive 95/46/EC (General Data Protection Regulation) (hereinafter: RODO), we inform you that:

1. The administrator of your personal data is the owner of the company BODEX-BHP Boguslaw Dolęga, whose registered office is located in Szczecin at Kopernika 4/7, 70-241, tel. 609-264-248,email: bodex.bhp@wp.pl
2. Your personal data will be processed for purposes related to:
   1. issuing certificates after the training or course,
   2. notification to the Office of Technical Inspection to conduct the state examination,
   3. Maintaining the documentation necessary for training and course records (Article 6(1)(a) of the DPA).
3. You are entitled to:
   1. access to their data,
   2. data rectification,
   3. limitation of data processing,
   4. data portability,
   5. The right to lodge a complaint regarding the processing of your personal data with the President of the Office for Personal Data Protection.
4. It is mandatory for you to provide your personal data, as this is regulated in the regulations on Technical Supervision and continuing education in non-school forms. The consequence of their failure will be the inability of BODEX-BHP Boguslaw Dolęga to provide training and courses, as well as to register participants for the examination conducted by the UDT. In other cases, the transfer of data is voluntary.
